THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Reperio Health, Inc. ("Reperio Health," "we," "us," or "our") is committed to protecting the privacy of your health and medical information.
Reperio Health offers two primary services: ReperioKit, an at-home biometric screening service, and ReperioCare, which includes ReperioKit together with a telehealth visit with a licensed clinician. Through both services, we collect individually identifiable health information, including demographic and biometric data. Individually identifiable health information that is subject to the Health Insurance Portability and Accountability Act ("HIPAA") is referred to as Protected Health Information ("PHI").
Please note that when we use the phrase, "biometric information," we mean statistical analysis of biological data, such as measurements of HDL or LDL cholesterol levels. We do not collect biometric information to identify a unique individual, such as a fingerprint, voiceprint, facial geometry or similar information.
For purposes of transparency and user protection, Reperio Health applies consistent privacy and security practices across its Services. This Notice of Privacy Practices ("Notice") describes how we may use and disclose health and medical information collected through both ReperioKit and ReperioCare, and outlines your rights with respect to that information.
When handling health and medical information, Reperio Health is required to:
– Maintain the privacy and security of your PHI;
– Provide you with this Notice of our legal duties and privacy practices;
– Follow the terms of the Notice currently in effect; and
– Notify you if a breach occurs that may compromise the privacy or security of your PHI.
This Notice applies to health and medical information created, received, maintained, or transmitted by Reperio Health in connection with its Services.
We may use and disclose your health and medical information without your authorization for the following purposes, as permitted by law.
We typically use and disclose your PHI for the following purposes:
Treatment
We may use and disclose your PHI to licensed clinicians and other health care providers involved in your care to provide, coordinate, or manage your health care through ReperioCare. For example, we may disclose information to a licensed doctor to interpret results collected using a ReperioKit.
Payment
We may use and disclose your PHI to bill and get payment from health plans or other entities. For example, we may provide information about you to your health insurance plan so it will pay for your services.
Health Care Operations
We may use and disclose PHI for our health care operations, including quality assessment, training, licensing, auditing, and internal administrative purposes necessary to operate and improve our services. For example, we may use your information to manage your treatment and services.

As Required by Law
We may disclose PHI when required to do so by federal, state, or local law.
Public Health and Safety
We may disclose PHI for public health activities, such as preventing or controlling disease, reporting adverse events, or responding to serious threats to health or safety.
Health Oversight Activities
We may disclose PHI to health oversight agencies for activities authorized by law, including audits, investigations, and inspections.
Research
We can use or disclose your PHI for health research.
Legal Proceedings and Law Enforcement
We may disclose PHI in response to court orders, subpoenas, or other lawful processes, and to law enforcement officials as permitted by law.
With Your Authorization
Other uses and disclosures of PHI not described in this Notice will be made only with your written authorization. You may revoke your authorization at any time in writing via the information in the Contact Information (Section 5) section below, except to the extent we have already relied on it.

Access Your Information
Request access to or a copy of your PHI maintained by Reperio Health in a designated records set.
Request Amendments
Request corrections to your PHI if you believe it is incorrect or incomplete.
Receive an Accounting of Disclosures
Request a list of certain disclosures of your PHI made by Reperio Health. However, this does not include any disclosures made directly to you, or disclosures to carry out treatment, payment and health care operations.
Request Restrictions
Request restrictions on certain uses or disclosures of your PHI. We are not required to agree to all requests but will review your request and comply where required by law.
Request Confidential Communications
Request that we communicate with you in a specific way or at a specific location.
Receive a Paper Copy of This Notice
Request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
File a Complaint
File a complaint if you believe your privacy rights have been violated. You may file a complaint with Reperio Health or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
We reserve the right to change this Notice and make the revised Notice effective for PHI we already maintain as well as any PHI we receive in the future. The current Notice will be posted on our website and will include the effective date.
